General Functions: A. Implementation 1. Assist the RN Supervisor in prioritizing family needs and responding appropriately to those needs.

2. Follow the plan of Care as written by the RN Supervisor. 3.

Perfom all procedures safely. 4.Documents actions and client response to care provided. 5.Actively listens to any concerns expressed by the client.

6.Proactive universal/standard precautions: • a. Proper handwashing • b. Contaminated laundry 7.Use equipment safely 8.Maintain client's comfort & well being.

B. Duties 1. Perform ADL's: • a.

Bathing • b. Dressing • c. Toileting • d.

Grooming • e. Oral Care 2. Meal Preparation & Feed Assistance 3.

Rehabilitation Exercises • a. Passive ROM • b. FROM 4.

Transfer, mobility assistance 5. Vital signs - B/P. Temperature, pulse, respiration, 6.

Assess and report change in condition status including temperature, respirations, blood pressure, mental status and alertness, illness (diarrhea, nausea, & vomitting etc....) 7. Performs other duties as assigned. C.

Reporting • Reports to RN Supervisor the client's response to care provided. • Assist the RN in revising the plan of care by objectively documenting and reporting client responses. • Assist the RN in revising short and long term goals for the client.

Assist and encourage the client in expressing any concerns or questions they may have. Responsible to: RN Supervisor Qualifications • Current unrestricted MD Nursing Assistant certification • Current CPR Certification • 1 year experience in the pediatric, hospital, nursing home, or home care environment in the past two years. • HS Diploma or equivalents.
Three references two professional, one character. • Criminal background check • Health Clearance- Annual physical exam, PPD or chest xray and Hep B clearance.
