Responsibilities • Deliver personalized patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts. • Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate documentation in electronic health records (EHR) systems like Epic or Cerner.

• Support patients with complex needs in diverse environments including long-term care facilities, hospitals, assisted living communities, and hospice settings. • Assist with medical procedures such as catheter care, medication administration under supervision, phlebotomy, and aseptic techniques to maintain infection control standards. • Observe and report changes in patient conditions promptly to nursing staff or physicians to ensure timely interventions.
• Maintain compliance with HIPAA regulations by safeguarding patient confidentiality during all interactions and documentation processes. • Use medical terminology accurately when documenting patient information and communicating with interdisciplinary teams. Skills • Proven experience working in various healthcare environments such as hospitals (including ICU, trauma centers, PICU), nursing homes, or outpatient clinics.
• Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate medical documentation. • Strong knowledge of physiology, anatomy, and medical terminology relevant to patient care across geriatrics, pediatrics, behavioral health, and acute care settings. • Experience providing dementia care, Alzheimer's support, memory care or working with individuals with developmental disabilities or disabilities.
• Ability to perform vital signs measurement accurately and execute basic life support (BLS) procedures confidently. • Skilled in using assistive devices such as Hoyer lifts or other mobility aids safely for patient transfers. • Knowledge of infection control practices including aseptic technique and infectious disease protocols in high-acuity environments like Level I Trauma Centers or Level II Trauma Centers.
• Excellent caregiving skills with a focus on compassionate patient service across home care, inpatient units or community-based settings.
