Job Summary We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to provide exceptional patient care in various settings, including assisted living communities, nursing homes, and home health environments. Duties • Observe and monitor patients’ conditions, including vital signs and overall health status, ensuring prompt communication of any concerns • Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts • Administer medications accurately following medical documentation protocols while maintaining HIPAA compliance • Support patients with dementia or Alzheimer’s disease through specialized memory care techniques and compassionate engagement • Provide medical care tasks including catheter care, wound dressing assistance, and hospice support as needed • Document patient information clearly • Prepare nutritious meals and assist with meal planning and preparation to promote health and well-being • Maintain a clean environment by assisting with light housekeeping tasks such as laundry and general cleaning to ensure a safe living space • Work effectively with individuals with developmental disabilities or disabilities by implementing personalized care plans • Use medical terminology appropriately when recording observations or communicating with healthcare teams Requirements • Valid CNA certification or HHA license required; experience in nursing homes or assisted living preferred • Knowledge of medical terminology, medication administration, and vital signs measurement • Experience working with individuals with dementia, Alzheimer’s disease, or developmental disabilities highly desirable • CPR certification required; additional certifications such as hospice care or catheter care are a plus • Familiarity with EMR/EHR systems for documentation; ability to accurately record patient data • Ability to work independently and as part of a multidisciplinary team in home & community care settings • Strong communication skills, patience, empathy, and a genuine desire to provide high-quality patient care • Physical ability to assist with patient mobility tasks using equipment like Hoyer lifts; capable of standing, walking, and performing caregiving duties for extended periods Join our team to make a positive impact through dedicated caregiving!
