We are expanding our clinical team to meet Medicaid requirements and better serve our growing client base.

Position Summary We are seeking a dedicated CNA/CMT who is compassionate, dependable, and skilled in delivering personal care and medication assistance. The CNA/CMT will work under the supervision of our Registered Nurse (RN) to provide hands-on support, follow individualized care plans, document client progress, and ensure a safe and supportive home environment. Key Responsibilities • Provide personal care assistance, including bathing, dressing, grooming, toileting, and mobility support • Assist with transfers, ambulation, and safe use of mobility devices • Administer medications as delegated by the RN in accordance with CMT certification • Monitor and report changes in client behavior, condition, or needs • Follow individualized care plans and update documentation daily • Perform light housekeeping tasks: meal prep, laundry, and cleaning related to client care • Assist with errands, companionship, and emotional support • Maintain strict confidentiality and comply with HIPAA and Maryland RSA regulations • Communicate effectively with the RN, clients, families, and agency staff • Participate in supervision meetings and skills check-ins as needed Qualifications • Active Maryland CNA certification (required) • Valid Maryland CMT certification (required) • Current CPR/First Aid certification • Reliable transportation and willingness to travel to client homes • Ability to pass background checks and pre-employment screening • Strong communication skills and professional demeanor • Compassion, patience, and a commitment to quality care • Experience working with seniors or individuals with disabilities is preferred, but not required What We Offer • Competitive pay and flexible scheduling • Supportive leadership and hands-on RN guidance • Opportunity to grow with a fast-growing home care agency • Meaningful work making a real difference for Maryland families How to Apply Interested candidates should email their resume to admin@centrycareplus.com or call 443-599-8280 to schedule an interview.
